Snapped or loose bolt

You’ve discovered a snapped bolt on the floor or one that’s simply loose.  Clearly this shouldn’t happen and there may be complex reasons for this occurrence that could include inadequate design or selection of bolts, poor installation, excessive material creep, vibrating machinery, fatigue, or overloading.  An inspection of the bolt and the structure is advised to gain insight and prevent further failures.
